For your first appointment, you'll need to fill out a registration form and any forms related to the service(s) you may need. Please arrive at least 10 minutes before your appointment to fill the forms, or you can do it in advance here. It is best to wear clothing that will allow easy access to the area(s) of discomfort. Your insurance card (in order to help you with your claims, or direct billing)
Your doctor’s note (only if you are required to have one)

YES, we do accept Motor Vehicle Accident (MVA) cases. If you are injured in a Vehicle Accident, please let us know when you’re booking your appointment. We require your claim and policy number, name of your adjudicator and your auto insurance company. In most cases, we can bill the insurer directly.
We are a registered provider with the Workplace and Safety Insurance Board (WSIB) of Ontario and can bill directly for services approved by WSIB. We will need to verify your case with WSIB prior to commencing treatment.
